Accessing Community Care in San Diego CA: The VA Community Care Network (CCN)

To schedule a Community Care Network appointment for Chiropractic care, please take the following steps to obtain a referral:

1.) GO TO THE VA:

• Schedule an appointment with your VA provider (doctor).
• Your VA care team will work with you to see if you are eligible for community care.
• Eligibility is based on your medical needs, care that is available at VA, and other requirements.

2.) MAKE AN APPOINTMENT

• The VA will work with you to create a chiropractic referral and schedule an appointment in our office.
  • Please note, that when a chiropractic referral is generated at the VA, they schedule directly based on geographic location. The provider is chosen automatically based on distance from the veteran's address. You do, however, have the option to choose your provider.
